CHAMPION ROUNDS PERFORMANCE PARENT/GUARDIAN CONSENT, ASSUMPTION OF RISK, RELEASE AND WAIVER

PROGRAM AND PARTICIPATION

Champion Rounds Performance is a non-contact athletic performance, conditioning, and boxing-inspired fitness program owned and operated by ZwebGenius LLC.

The program is designed to improve athletic skills such as conditioning, strength, stamina, coordination, agility, footwork, speed, balance, movement, and overall athletic performance.

Training may include heavy-bag work, coach-held mitt or pad work, jump rope, plyometrics, resistance-band exercises, agility drills, bodyweight exercises, strength and conditioning exercises, movement drills, and other supervised fitness activities.

Champion Rounds Performance is NON-CONTACT.

There is no participant-to-participant sparring. Participants are not instructed or permitted to intentionally punch, kick, or strike another participant.

ASSUMPTION OF NORMAL ATHLETIC RISK

I understand that even though Champion Rounds Performance is a non-contact program, physical exercise and athletic training involve normal and inherent risks.

These risks may include muscle soreness, strains, sprains, bruises, falls, overexertion, accidental contact with exercise equipment or another participant, aggravation of an existing condition, and other injuries that may occur during physical exercise or athletic training.

I understand that not every possible risk can be predicted or completely eliminated.

I voluntarily authorize my child to participate in Champion Rounds Performance and understand and accept the inherent risks associated with physical training.

HEALTH AND SAFETY

I agree to provide accurate information regarding any known injury, medical condition, physical limitation, allergy, or other condition that may reasonably affect my child’s participation.

I understand that Champion Rounds Performance and its coaches do not provide medical diagnosis or medical treatment.

My child is expected to follow coaching instructions, facility rules, and reasonable safety directions.

My child should stop participating and inform a coach if they experience unusual pain, dizziness, difficulty breathing, illness, injury, or another condition that may make continued participation unsafe.

EMERGENCY ASSISTANCE

If an apparent emergency occurs, I authorize Champion Rounds Performance staff or coaches to provide reasonable first-aid assistance, contact the emergency contact listed in the registration, and contact emergency medical services when reasonably believed necessary.

I understand that this authorization does not create a guarantee or obligation that medical services will be available or provided.

RELEASE AND WAIVER

To the fullest extent permitted by Ohio law, I, as the participant’s parent or legal guardian, voluntarily assume the inherent risks associated with my child’s participation in Champion Rounds Performance.

To the fullest extent permitted by applicable law, I release and waive claims arising from participation against ZwebGenius LLC, Champion Rounds Performance, and their owners, officers, employees, coaches, instructors, contractors, representatives, and agents, including claims arising from ordinary negligence.

This release does not apply to claims or liability that cannot legally be waived or released under Ohio law.

This agreement applies to the minor participant identified in this registration and to participation in Champion Rounds Performance activities.

THIRD-PARTY FACILITY

Champion Rounds Performance may conduct training at a separately owned or operated facility.

Use of a third-party facility may be subject to that facility’s own separate rules, policies, terms, and waiver.

Any third-party facility waiver is separate from this Champion Rounds Performance agreement.

Champion Rounds Performance does not modify or replace the legal terms of a third-party facility.

PARENT/GUARDIAN AUTHORITY

By signing this agreement, I represent that I am the parent or legal guardian of the minor participant identified in this registration and that I have authority to authorize the minor’s participation.

I have had the opportunity to review this agreement before signing.

ELECTRONIC CONSENT AND SIGNATURE

At the bottom of the agreement, the parent/guardian provides one mandatory consent and an electronic signature.
